Behavioral Health Issues Clinical Pathway — Emergency Department

Children Presenting to Triage with Primary BH Chief Complaint

Age < 18 yrs
  • Sign-in and triage as usual
  • Contact BHC Leader, assign safety observer as indicated immediately
Age ≥ 18 yrs
  • Sign-in and triage as usual
  • ED triage RN or PAC will notify SW to review history
  • SW reviews with ED Attending APOD and PHL ED Psych
    • Keep child in triage booth under staff observation or in waiting with 1:1 observer
  • Post-review Disposition
    • Admit to CHOP ED for further care if:
      • Triage huddle identifies medical concern and < 19 yrs
      • Currently followed by CHOP BH Team
      • Presence of neurodevelopmental issues that may benefit from pediatric subspecialty
    • For children not meeting criteria:
      • PEM Attending completes MSE, stabilization as indicated
      • RN completes triage assessment
      • Call HUP, obtain accepting MD, provide hand-off CHOP staff will transport child
      • Provide printed chart to HUP
      • Consider need for security for transfer process
