Goals

  • Increase the identification and documentation of weight assessment according to BMI percentile at every well visit
  • Increase appropriate use of laboratory testing
  • Increase weight management follow-up for children with BMI ≥85th percentile in medical home

Metrics

BMI Documentation

  • BMI ≥85-94%
    • Increase documentation of this in the diagnosis or problem list at every well visit to 50%
  • BMI ≥95%
    • Increase documentation of this in the diagnosis or problem list at every well visit to 75%
      • BMI ≥95% - 98%: Obese, Obesity
      • BMI ≥99%: Severe Obesity

Laboratory Studies

  • Increase to 75%: Recommended labs ordered and resulted in the preceding 2 years
  • Increase to 50%: Follow-up within 6 months of patients with abnormal labs

Weight Management

  • Increase to 50%: Patients with BMI > 85% with follow-up weight management at least every 6 months
