- Increase appropriate use of laboratory testing
- Increase weight management follow-up for children with BMI ≥85th percentile in medical home
Metrics
BMI Documentation
- BMI ≥85-94%
- Increase documentation of this in the diagnosis or problem list at every well visit to 50%
- BMI ≥95%
- Increase documentation of this in the diagnosis or problem list at every well visit to 75%
- BMI ≥95% - 98%: Obese, Obesity
- BMI ≥99%: Severe Obesity
- Increase documentation of this in the diagnosis or problem list at every well visit to 75%
Laboratory Studies
- Increase to 75%: Recommended labs ordered and resulted in the preceding 2 years
- Increase to 50%: Follow-up within 6 months of patients with abnormal labs
Weight Management
- Increase to 50%: Patients with BMI > 85% with follow-up weight management at least every 6 months
