Lower-risk Infant and Considerations for Management

Lower-risk Criteria
  • Age > 60 days
  • Gestational age
    • Born ≥ 32 wks gestation and post-conception age ≥ 45 wks
  • No CPR by trained medical provider
  • Event lasted < 1 min
  • First event
  • No repeat events
  • No concerning history
Considerations for Management
  • Infants who meet lower-risk BRUE criteria have a lower risk of having a recurrent event, serious underlying disorder, or sudden death.
  • Laboratory studies, imaging studies, other diagnostic procedures, or hospitalization are generally unnecessary.
  • In a recent multi-center study comprising > 2,000 subjects, of 861 ECGs done, none contributed to a new diagnosis. Other diagnostic studies were also of low yield and rarely contributed to an explanation.
