Weaning IV Dextrose Treatment

For familial history of hypoglycemia, consult Endocrinology for weaning plan.

Begin Wean Once BG > 60 mg/dL Recommend 2-3 values > 60 mg/dL prior to initiating wean
Weaning Strategy
  • BG > 60 mg/dL
    • 1 ml/hr - GIR wean of 0.5
  • BG > 70 mg/dL
    • 2 ml/hr - ˜GIR wean of 1
Monitor Off IV Dextrose for
2-3 checks
  • May discontinue BG checks if
    • BG > 50 mg/dL when < 48 hrs of age
    • BG > 60 mg/dL when ≥ 48 hrs of age
