Neonates with Hyperbilirubinemia/Jaundice Clinical Pathway
Home LED-based Phototherapy
- Infants with TSB no greater than 1 mg/dL above the phototherapy threshold can be treated with home phototherapy if all the below conditions are true:
- Gestational age > 38 weeks
- Infant is > 48 hours old
- Clinically well, adequate feeding
- No known neurotoxicity factors
- No previous phototherapy
- No delay in device
- TSB can be measured daily

- CBC
- Retic
Monitoring
- Measure TSB daily
Admission for Phototherapy
- TSB increases and the difference between the TSB and the phototherapy threshold narrows
- TSB is ≥ 1 mg/dL above the phototherapy threshold
As with inpatient phototherapy, it is an option to start home phototherapy at a lower threshold (e.g., 2 mg/dL below the phototherapy threshold) to reduce the readmission risk
