Neonates with Hyperbilirubinemia/Jaundice Clinical Pathway

Home LED-based Phototherapy

  • Infants with TSB no greater than 1 mg/dL above the phototherapy threshold can be treated with home phototherapy if all the below conditions are true:
    • Gestational age > 38 weeks
    • Infant is > 48 hours old
    • Clinically well, adequate feeding
    • No known neurotoxicity factors
    • No previous phototherapy
    • No delay in device
    • TSB can be measured daily

  • CBC
  • Retic

Monitoring

  • Measure TSB daily

Admission for Phototherapy

  • TSB increases and the difference between the TSB and the phototherapy threshold narrows
  • TSB is ≥ 1 mg/dL above the phototherapy threshold

As with inpatient phototherapy, it is an option to start home phototherapy at a lower threshold (e.g., 2 mg/dL below the phototherapy threshold) to reduce the readmission risk
