Infant Malnutrition Clinical Pathway — Outpatient Specialty Care and Primary Care

Goals

  • Reduce variability of care process and more consistent approach to the evaluation and management of infants with malnutrition
  • Early recognition of malnutrition in infants, efficient diagnosis, and management of malnutrition in the outpatient setting including timely referral as appropriate to specialist, clinical nutrition, social worker, behavioral health
  • Prevent unnecessary referral to ER or hospital admission and indirectly decrease utility cost

Metrics

  • Improve % of infants with adequate weight gain and growth
  • Reduction of labs and radiology studies
  • Reduction in usage of Failure to Thrive as a diagnosis
  • Reduction in ER visits/hospitalizations
  • Reduction in cost
