Infant Malnutrition Clinical Pathway — Outpatient Specialty Care and Primary Care

Cohort

This pathway applies to infants < 12 mos currently being seen in outpatient primary care and specialty clinics with the following signs, symptoms, and diagnosis: poor weight gain or weight loss, which may or may not include malnutrition diagnoses.

Failure to thrive (FTT) is a symptom, not a diagnosis. Malnutrition is a diagnosis that more accurately describes inadequate nutrition and can be determined via clinical assessment.

Exclusion Criteria

  • Premature infants < 37 wks
  • Infants older than 12 mos of age
  • Infant with no risk for malnutrition
  • Infants currently hospitalized
  • Known genetic, metabolic condition, or infants with known chronic disease (e.g., celiac disease, IBD, CF, chronic renal disease, etc.) already followed by subspecialist (should be referred back to specialists)
  • Infants currently tube fed
