Malnutrition, Weight Loss and Eating Disorders Clinical Pathway — ICU and Inpatient

Consults

Clinical Nutrition
  • Determine caloric advance to reach mid-point goal calories
  • Acquire and interpret growth curves to determine pre-morbid growth trajectory based on BMI
  • Calculate and educate parents on initial goal weight range based on patient’s current age
  • Recommend additional labs or supplements per clinical judgment
  • Provide education to families on meal plan and planning
Behavioral Health
  • Recommend level of observation (discuss in rounds)
    • If no concerns of self-injury, options include 1:1 for meals/snacks/rest, MOORS, and/or parent observers
    • Safety assessment as needed to determine if 24 hr 1:1 is needed
  • Guides formulation and implementation of behavioral health plan when needed, including engagement of BCBA
  • Formulate and communicate Behavioral Health diagnosis when present
    • Recommend behavioral health disposition including modality and level of care
Social Work
  • Psychosocial assessment and determination of basic needs
  • School communication and accommodations
  • Communication with involved outside social service and behavioral health agencies
  • Support family in addressing health insurance needs
  • Support families in accessing recommended Behavioral Health level of care
Child Life, Creative Arts Therapies, and Integrative Health Services
  • Together these three services provide individualized and small group support for coping and trying new coping strategies
  • Child Life supports the teen room and play deck and can work with patients to develop a daily schedule
  • Creative Arts Therapies include Music, Dance/Movement, and Art Therapists and offer individual and group sessions
  • Integrative Health offers a wide range of therapies including aroma and tactile therapies, yoga, ad acupuncture
