Malnutrition, Weight Loss and Eating Disorders Clinical Pathway — ICU and Inpatient

Discharge Criteria

Consider the following as minimal criteria for hospital discharge:

Vital Signs
  • Heart rate
    • ≥ 50 beats/min during the day
    • ≥ 46 beats/min at night for 48 hours
  • Blood pressure
    • ≥ 90/50
  • Resolved orthostatic dizziness and orthostasis by BP
Anthropometrics
  • > 75% of BMI preferred, particularly if discharged to family-based treatment (FBT)
    • Discuss with outpatient team prior to discharge at lower BMI
  • Reaching goal calories and gaining weight
Labs
  • Electrolyte stability
    • BMP/Mag/Phos within normal limits without supplementation
  • No further concerns for refeeding syndrome
Disposition Recommendation Formalized
  • For those discharged to home, caregivers have had an opportunity to choose and supervise meals
  • If family rejects disposition recommendations: Alternative outpatient plan including coordination with PCP and other services as needed
