Indications for a Partial Volume Exchange Transfusion in Neonates
This pathway should be used to guide the management of infants requiring a PVET using the following criteria:
- Symptomatic anemia with euvolemia and/or cardiac dysfunction
- Symptomatic polycythemia hyperviscosity syndrome
- Asymptomatic polycythemia with hematocrit > 75%
- PVETs should only be performed in the NICU/CICU/PICU
Exclusions
- Risks of procedure outweigh the benefits of procedure
- Patients requiring a double volume exchange
Defining Partial Volume Exchange Transfusion
- Procedure that introduces donor blood or other fluids in exchange for variable percentage of circulating blood volume depending on degree of anemia/polycythemia
- The infant’s blood is repeatedly withdrawn in small amounts and replaced with equal amounts of donor blood/other fluids
- Purpose is to correct extremes of hematocrit while maintaining and/or achieving euvolemia
