Partial Volume Exchange Transfusion, Clinical Pathway, ICU

Partial Exchange Volume/Passes/Frequency

Definition Notes Example
Total Volume of Blood to Exchange Determined using equations specific to anemia or polycythemia 75 mL
Length of Procedure In general, PVET should be done over 30-45 minutes based on volume of blood being exchanged, hemodynamic stability of patient and access. Hemodynamic instability and/or peripheral access are considerations for using a longer length of exchange of 60-90 minutes. 30 minutes
Length and Volume of Each Pass
A pass is the amount withdrawn over a set interval of time.
Use of passes ensures that the amount being withdrawn is equal to the amount being infused. Consider using 3-5 minutes as the pass time. 30 minutes for entire exchange/5 minute pass time = 6 passes

75 mL/6 = 12.5 mL to be withdrawn over 5 minutes
Pump Rate
Rate at which nursing will program the infusion pump in mL/hour
  75 mL/30 minutes = 2.5 mL/min x 60 = 150 mL/hr
Volume to be Infused (VTBI) Total volume entered into “VTBI” part of infusion pump by RN at start of each pass 12.5 mL
