Newborn/Infant Perioperative Care Clinical Pathway — N/IICU

Preoperative Laboratory Studies

The following lab tests are recommended for infants before surgery. Other tests might be indicated based on clinical scenario.

  • Consider addition of IV Vitamin K to TPN or subcutaneous 1-2 mg daily for 72 hrs, especially if undergoing abdominal surgery for:
    • Conjugated bilirubin > 2 mg/dL
    • Elevated LFTs
    • Infant is coagulopathic
Laboratory Study Indications/Frequency
Basic Metabolic Panel
  • 24-48 hrs before surgery for patients on diuretics, electrolyte supplements, TPN
  • Obtain within 7 days before surgery for all other patients
Type and Screen (T&S)
  • Infants < 120 days old: confirm active T&S in Blood Bank
  • Infants ≥ 120 days old: send new T&S
    • New T&S valid for only 72 hrs
  • May need blood type confirmation, if not obtained previously
CBC with Differential 24-48 hrs before surgery
