Persistent Hypoglycemia, Newborn, N/IICU — Additional Evaluations to Consider — Clinical Pathway: ICU

Newborn Persistent Hypoglycemia Clinical Pathway — N/IICU

Additional Evaluations to Consider

Genetic Testing
Cardiology Consult
  • Newborns with hyperinsulinism can have hypertrophic cardiomyopathy
    • Excess insulin can cause RVH, infants prone to fluid overload
  • Consider baseline ECHO in neonates with KATP HI or those born SGA
  • For neonates on diazoxide, obtain ECHO one week after starting diazoxide or prior to discharge
Genetics Consult
  • Possible syndromic causes of hyperinsulinism
  • Abdominal U/S, AFP level in neonates with suspected Beckwith-Wiedemann
PET Scan (CHOP)

Patients with hyperinsulinism

  • Imaging by 18F-Dopa
  • Requires general anesthesia, takes about 2 hours
  • This is an IND-research study
  • Used to localize focal lesion in the pancreas
