Anesthesia/Sedation Preoperative Care and Preparation Clinical Pathway, Inpatient and Emergency Department

Parenteral Nutrition (PN) Perioperative Considerations

  • Stop IV lipid emulsions prior to general anesthesia/sedation
  • Evaluate risk of post-operative stress-induced hyperglycemia when selecting perioperative fluids
  • Cycled PN – communicate cycle times to procedure area
Population Age Peri-Op Considerations Comments
< 1 yr Refer to Perioperative Care, Newborn/Infants Clinical Pathway Convert to equivalent dextrose-containing
IV fluid, such as 1/2NS D10, prior to going to OR/Radiology
≥ 1 yr
  • Continue PN as ordered and maintain through transport to procedure area
  • Ensure trifuse connector is primed and one lumen remains open for line access in OR
  • Severe liver dysfunction (e.g., short-gut), Altered endocrine function (e.g., hyperinsulinism), at risk for hypoglycemia, metabolic disorders, and chronic PN
  • Obtain CMP within 24 hrs prior to procedure
