Pulmonary Embolism Response Team (PERT) Activation

PERT will assist with risk stratification, recommend therapy and guide patient disposition for intermediate and high-risk PE. Hematology will consult for low-risk PE.
Unstable Hemodynamics
  • PICU Consult, CAT, CODE or ED Tier
  • PICU will activate PERT
Stable Hemodynamics
  • Hematology Consult
  • Hematology will activate PERT for intermediate and high-risk PE

Referring Clinician Should Prepare the Following Data for PERT Consultation

HPI, Clinical stability
  • Brief history of event
  • Active CPR, current VS, vasoactive medications
Labs
  • CBC
  • PT
  • PTT
  • Fibrinogen
  • Cr
  • BNP
  • Troponin
Imaging
  • CTA and echo if done
tPA Contraindications
