Therapeutic Range
Therapeutic dose goal PTT: 60-85 seconds, anti-Xa: 0.3-0.7 units/mL

TPA

Loading Dose
  • Heparin 75 units/kg IV over 10 min (max dose 5000 units)
  • Use caution or avoid bolus in
    • Sick/premature neonates
    • Stroke
    • Active bleeding or high risk for bleeding
Maintenance Dose
  • < 1 yr: 28 units/kg/hr
  • 1 to < 12 yrs: 20 units/kg/hr, max initial dose 1,333 units/hr
  • ≥ 12 yrs: 18 units/kg/hr, max initial dose 1,333 units/hr
