Suicide Risk Assessment and Care Planning Clinical Pathway — Outpatient Specialty Care
Goals
To standardize the care practices for patients at risk for suicide by:
- Increasing the reliability of identification of suicidal ideation and behavior through screening
- Providing guidance and support to clinicians to complete a full suicide risk assessment by integrating the presence of suicidal ideation, behavior, risk and protective factors
- Determining risk formulation using the assessment data
- Deciding on the appropriate immediate and ongoing plans of care in collaboration with the patient and family
- Supporting documentation and enhancing communication among clinical teams about a patient’s suicide risk status and care needs
Metrics
- Number of patients who screened positive for suicide risk and has C-SSRS complete/number of patients with positive screen
- Number of patients who are placed on the pathway/number of patients who meet criteria for being placed on the pathway
- Number of patients with a completed safety plan/number of patients who meet criteria for needing a safety plan
