Suicide Risk Assessment and Care Planning Clinical Pathway — Outpatient Specialty Care

Goals

To standardize the care practices for patients at risk for suicide by:

  • Increasing the reliability of identification of suicidal ideation and behavior through screening
  • Providing guidance and support to clinicians to complete a full suicide risk assessment by integrating the presence of suicidal ideation, behavior, risk and protective factors
  • Determining risk formulation using the assessment data
  • Deciding on the appropriate immediate and ongoing plans of care in collaboration with the patient and family
  • Supporting documentation and enhancing communication among clinical teams about a patient’s suicide risk status and care needs

Metrics

  • Number of patients who screened positive for suicide risk and has C-SSRS complete/number of patients with positive screen
  • Number of patients who are placed on the pathway/number of patients who meet criteria for being placed on the pathway
  • Number of patients with a completed safety plan/number of patients who meet criteria for needing a safety plan
