Analgesia and Sedation for Extremity Fracture Care

  • Optimize pain management before X-ray, splint removal when possible
    • Ice, elevation
    • Ibuprofen if not given in triage
    • Fentanyl 1 – 2 mcg/kg (max 100 mcg/dose) IN is effective for analgesia before radiographs; IV not required
    • IV fentanyl or morphine, if IV in place, can be given before radiographs and to promote comfort as needed
    • IV diazepam (0.04-0.1mg/kg/dose, max 5 mg) may be required for muscle spasms to assist with placing traction
    • Discuss with Ortho hematoma block when appropriate
