Analgesia and Sedation for Extremity Fracture Care
- Optimize pain management before X-ray, splint removal when possible
- Ice, elevation
- Ibuprofen if not given in triage
- Fentanyl 1 – 2 mcg/kg (max 100 mcg/dose) IN is effective for analgesia before radiographs; IV not required
- IV fentanyl or morphine, if IV in place, can be given before radiographs and to promote comfort as needed
- IV diazepam (0.04-0.1mg/kg/dose, max 5 mg) may be required for muscle spasms to assist with placing traction
- Discuss with Ortho hematoma block when appropriate
