Criteria for Empiric Antibiotic Treatment

Empiric antibiotic treatment is defined as antibiotics administered to children based on suspicion of UTI before culture results being available.

Population Recommendations
2-23 Months
  • Use UTICalc   with child’s risk factors + POC urine dipstick
  • Results, treat empirically if high probability of UTI
≥ 24 Months
  • Consider empiric treatment based on compatible history and physical exam and either:
    • Leukocyte esterase ≥ 2+ or nitrite + on urine dip
    • or
    • > 5 WBC/hpf and bacteriuria
