Vascular Access Clinical Pathway — Inpatient
Midline Catheter Guidelines
A midline catheter is not a substitute for central venous catheter placement.
Eligibility Criteria
- Weight > 10 kg
- Only IV fluids and medications appropriate for peripheral access
- Non-irritating fluids for > 72 hrs
- PPN > 72 hrs
- Transfusions > 48 hrs
- Blood draws > 48 hrs
- Anticipated duration ≤ 14 days
- History of difficult vascular access
Exclusion Criteria
- TPN
- High-concentration inotropes or electrolytes (> 10% dextrose, > 600 mOsM)
- Vesicants/irritants
- Burns, infection, contractures, wounds, thrombus in extremity, or compromised blood flow to insertion extremity
- Concerns for, or diagnosed chronic renal insufficiency for the purpose of vessel preservation
- Anticipated need for repeated PICCs in the future
