Venous Thromboembolism (VTE) Prevention Clinical Pathway — Inpatient

Goals

  • VTE risk assessment is completed for every inpatient ≥ 12 years.
    • On admission
    • At transfer
    • With any significant change in medical condition
    • Every 24 hours
    • Perioperatively
  • The appropriate VTE prevention intervention, based on risk level, is applied to all applicable inpatients.
  • To ensure that sequential compression devices are placed on all patients ≥ 12 years of age who are having a surgical procedure with general anesthesia that lasts ≥ 1 hour.

Metrics

  • Percent compliance with VTE risk assessment during hospitalization
  • Percent compliance with the appropriate VTE prevention based on risk level
  • Percent compliance with the use of SCDs during surgical procedures
  • VTE events that are not central line-associated in children ≥ 12 years of age
