Venous Thromboembolism (VTE) Prevention Clinical Pathway — Inpatient
Goals
- VTE risk assessment is completed for every inpatient ≥ 12 years.
- On admission
- At transfer
- With any significant change in medical condition
- Every 24 hours
- Perioperatively
- The appropriate VTE prevention intervention, based on risk level, is applied to all applicable inpatients.
- To ensure that sequential compression devices are placed on all patients ≥ 12 years of age who are having a surgical procedure with general anesthesia that lasts ≥ 1 hour.
Metrics
- Percent compliance with VTE risk assessment during hospitalization
- Percent compliance with the appropriate VTE prevention based on risk level
- Percent compliance with the use of SCDs during surgical procedures
- VTE events that are not central line-associated in children ≥ 12 years of age
