Confirmed Withdrawal

If Opioid Most Recently Weaned

  • Administer ordered PRN opioid dose equivalent to 50% of the current enteral standing dose
  • If the patient is on a continuous opioid infusion, give an opioid bolus dose equivalent to 50% of the current hourly dose
  • If WAT score is greater than goal despite initial rescue, give a rescue dose of benzodiazepine equivalent to 50% of the current enteral standing dose (if ordered)

If Benzodiazepine Most Recently Weaned

  • Administer ordered PRN benzodiazepine dose equivalent to 50% of the current enteral standing dose
  • If the patient is on a continuous benzodiazepine infusion, give a benzodiazepine bolus dose equivalent to 50% of the current hourly dose
  • If WAT score is greater than goal despite initial rescue, give a rescue dose of opioid equivalent to 50% of the current enteral standing dose (if ordered).

If more than 3 rescue doses are required in a 24-hour period, a new wean plan may be necessary.
