Transfer Back to Referring Hospital

  • Transfer back with reservoir in place can be considered on an individual patient basis. Individual patient issues, stability and the comfort of the receiving site and neurosurgery will need to be considered.
  • Add a note to the electronic bulletin board that patient should receive care in a high-risk follow-up clinic
  • Add concerns for high pressure to the discharge summary

Recommended Follow-up Care

  • See Neurosurgery for follow-up within one month of transfer back to the referring hospital (215-590-2780)
  • Initiate ongoing care in a High-risk Follow-up Clinic locally or at CHOP (215-590-2183)
  • Register for Early Intervention services
  • Routine newborn follow-up with the primary pediatrician
