Definition of Persistent Hypoglycemia

It is difficult to give an absolute level of plasma glucose, dextrose requirement or age at which to consider further evaluation, however diagnostic work-up should be considered in infants < 12 months of age with hypoglycemia:

  1. Severe hypoglycemia (e.g. episode of symptomatic hypoglycemia)
  2. Inability to consistently maintain pre-prandial plasma glucose concentrations > 50 mg/dL up to 48 hrs. of life and > 60 mg/dL after 48 hrs of life
  3. Persistently requires ≥ 8-10 mg/kg/min for 3-10 days to maintain serum glucose > 60
  4. Family history of a genetic form of hypoglycemia
  5. Congenital syndromes (e.g., Beckwith-Wiedemann), abnormal physical features (e.g., midline facial malformations, microphallus)
