Neuromuscular Compromise and Respiratory Failure — MI-E (CoughAssist) Settings — Clinical Pathway: Inpatient and PICU

Neuromuscular Compromise and Respiratory Failure Clinical Pathway — Inpatient and PICU

MI-E (CoughAssist) Settings

For new initiation of a patient naïve to therapy, begin with set pressures of:

  • Pi: start at 20 cmH2O and increase to see visible chest rise
  • Pe: start at -20 cmH2O and adjust for secretion removal

For invasively ventilated patients, start at a peak inspiratory pressure (PIP) or Pi of 20 cmH2O, whichever is higher.

Perform 5 sets of 5 cycles:

  • Inhale time = 1-3 second(s)
  • Exhale time = 1-3 second(s)
  • Pause time = 2 seconds (1 second if patient is < 1 year)

Bleed in oxygen to MI-E (CoughAssist) if requiring supplemental oxygen.
