Glucose Monitoring in the Healthy Newborn Clinical Pathway — Inpatient

Safety Fast Indications

  • History of symptomatic hypoglycemia
  • Prolonged requirement for IV dextrose (3-7 days)
  • High GIR requirements (> 8 mg/kg/min)
  • Use of Glucagon

Safety Fast Instructions

Start of Fast
  • Check pre-feed glucose at normally scheduled feeding time
  • Make NPO immediately following feed
During Fast
  • Check blood glucose 3 hrs after start of fast feed
    • Then every hour until the 6 hr mark post start of fast feed
    • Maximum of 3 additional glucose checks
End of Fast
  • If 6 hr mark post the last feed is reached
  • or
  • If glucose is < 65 mg/dL, repeat for confirmation
  • Give feed
Interpretation of Fast Pass = Glucose > 65 mg/dL throughout fast

Safety Fast Example

Fast Time Actual Time Glucose Feeds
0 hr (pre-feed) 9 a.m. 90 mg/dL 60 ml formula
3 hr 12 p.m. 85 mg/dL NPO
4 hr 1 p.m. 77 mg/dL NPO
