Is your child at risk of aggression towards themselves or others during dentist visits or haircuts when someone touches their head? Yes, Major Concerns
Does your child have difficulty participating in situations like dentist visits or haircuts in which someone is touching their head? Yes, Minor Concerns
Has your child had difficulty completing a prior EEG? Yes, Minor Concerns

Major Concerns	Outpatient EEG Direct family to Visual Support Tool and EEG Information Prescribe pre-medication for family to give before EEG Elective Inpatient EEG Monitoring Direct family to EMU Information Recommend pre-medication for inpatient team Order will notify child life specialist for planning Urgent/Emergent Inpatient EEG Prescribe pre-medication Consult child life specialist or BHIP as needed
Minor Concerns	Outpatient EEG Direct family to Visual Support Tool and EEG Information Consider prescribing pre-medication for family to give before EEG Elective Inpatient EEG Monitoring Direct family to EMU Information Consider pre-medication for inpatient team Order will notify child life specialist for planning Urgent/Emergent Inpatient EEG Consider prescribing pre-medication, Child Life, BHIP as needed
