Psychosocial Assessment
  • Consider depression and anxiety screen if:
    • Acute or chronic stressors
      • e.g., trauma, major life change, bullying
    • Physical complaints suggestive of a functional cause
    • Positive PHQ-9 or BHS screen
    • History of a psychiatric diagnosis
      • e.g., ADHD, anxiety
    • A decline in overall behavioral/emotional functioning
    • Recent suicidal ideation or behavior
    • Family history of depression
    • Legal/juvenile justice involvement

  • Elopement concern
  • Suicidal ideation

If Adolescent is at Risk for Elopement or for Self-harm
  • Consult FAQs Elopement, Abduction, Missing Patients and Code Amber Job Aid
  • Scenarios for higher risk include:
    • Court-ordered substance use treatment via outpatient care
    • If DHS is involved due to substance use and elopement
    • If eloped off the unit before withdrawal is well-managed
  • To reduce risk of elopement:
    • Discuss adolescent's risks if attempts to elope on daily rounds
