Psychology and Social Work Perspectives on Transition to Adult Care

This webinar reviews common barriers to a smooth transition to adult care and how psychology can help. You’ll learn ways to slowly increase your involvement in your care, including medical decision-making, organizing your medications and follow-up appointments, and overall self-management of your health and well-being.
