Psychology and Social Work Perspectives on Transition to Adult Care

This webinar reviews common barriers to a smooth transition to adult care and how psychology can help. You’ll learn ways to slowly increase your involvement in your care, including medical decision-making, organizing your medications and follow-up appointments, and overall self-management of your health and well-being. We know that having a chronic medical condition is stressful and that taking care of yourself involves more than just taking your medications, so this webinar also reviews some ways to manage that frustration and offers recommendations for recognizing when working with a psychologist might be helpful.
