Preparing For Your Endocrinology Appointment

To ensure your child’s visit is as complete and thorough as possible, please make sure all the following information is obtained before the office visit. Missing information may result in additional testing and office visits. If you have any questions, please contact us at 215-590-3174.  

Before your appointment

Medical record

Before your child’s appointment with the Division of Endocrinology and Diabetes, please obtain the following information from your child’s referring/primary care provider. Please send the documents by email to endomedicalrecords@chop.edu or fax 215-590-3053.

  • Growth chart
  • Most recent office notes
  • All lab tests from the past year
  • If previously seen by another endocrinologist, provide medical summary/records/letters

If your child is being evaluated for delayed puberty, early puberty or short stature and is over 5 years old, a bone age X-ray is important. If your child had a bone age outside of CHOP, please bring:

  • Plain film or a CD of the bone age X-ray

  • Radiology report describing the X-ray results

If your child has not had a bone age X-ray, please ask your referring/primary care provider to order one to be completed before your child’s visit.

Forms

Complete the New Patient Intake Form using MyCHOP before coming to the office. Visits may be delayed if the intake form has not been completed. If you have not created your MyCHOP login and you need a MyCHOP access code to get started, contact our office at 215-590-3174.

Insurance and referral information

If your insurance plan requires a referral from your referring/primary care provider, please be sure to obtain one before your child’s visit (two weeks before the visit for non-participating plans). Please check with your insurance company about covered benefits for the specialty provider visit and testing at CHOP. In addition, you should ask your insurance about any out-of-pocket expenses, such as co-pays or deductibles, that might be due during the visit. You may be responsible to pay a co-pay or deductible for both your child’s physician visit and testing or treatment service.

Day of your appointment

Please arrive 30 minutes before your scheduled appointment for registration, to review insurance information, and to allow time for triage before being seen by your provider. If your child is being seen in the Buerger Center for Advanced Pediatric Care, please allow extra time for traffic, parking and check-in.

Remember to bring your current insurance card with you. If you need to cancel your appointment, please provide at least 72 hours’ notice. Please be aware that if you arrive late or without required paperwork, your appointment may be delayed or rescheduled.
