STRIVE for Kids Program Resources for Professionals
Once you have identified a patient you would like to be evaluated by CHOP’s STRIVE for Kids Program, please inform the patient/patient family that you would like to refer them to us for further assistance.
Based on the patient/family interest and insurance approval, there are two scheduling options:
- Referring clinicians from CHOP can order an Endocrine or Roberts Individualized Medical Genetic Center (IMGC) consult in EPIC. Please specify your indent that the child be evaluated by the STRIVE for Kids Program.
- External referring clinicians can initiate contact by calling 267-426-7418 or emailing striveforkids@chop.edu.
Our team sees patients once a month on the second Wednesday for scheduled visits.
If you are interested in our clinic and learning how we may be able to help your patient, please contact us. We will work to get your patient scheduled as soon as possible.
Before the first visit
Before the child’s first scheduled visit with CHOP’s STRIVE for Kids Program, we will need the referring provider or family to send us all related medical records and X-rays. These materials may be emailed, faxed or sent by postal mail.
