Glucose Monitoring in the Healthy Newborn Clinical Pathway — Inpatient

IV Dextrose Treatment Steps

Bolus with D10 2ml/kg As needed to achieve BG > 60 mg/dL
Begin D10W at 60-80 ml/kg/day —
Increase TFL/Dextrose Concentration as Needed
  • Maintain BG > 60 mg/dL while on IV dextrose
  • Continue feeds per unit guidelines
Evaluate Need for Central Line
  • Required for all IVF with greater than D12.5 concentration
  • Consider when approaching TFL of IVF 100-120 mL/kg/day
Consider Glucagon Infusion
  • High GIR requirements (> 10 mg/kg/min)
  • Concerns for fluid overload
Consider Endocrinology Consult,
Refer to Persistent Hypoglycemia in the Newborn Pathway		 If persistent IVF requirement and/or inability to wean
