Infant Malnutrition/Failure to Thrive (FTT) Clinical Pathway — Inpatient

Patient Family Education

Coordinate discharge requirements as needed; some caregivers will require education, demonstration of skills, and supplies.

Caregiver Demonstration

  • 24 hour stay: May be required if nursing concerns indicate need for additional training and practice
  • Orally fed patients: Caregiver demonstrates safe and effective feeding techniques
  • Tube-fed patients: Caregiver inserts tube on the child successfully with minimal nursing support

Supplies

  • If formula, WIC paperwork (Provider)
  • If specialty formula needed (Case Management)
  • Tube-fed patients:
    • Order NG supplies (and pump if necessary) and ensure delivery to bedside or home (Case Management)
    • Update home care company if previous plan of care changes (Case Management)
    • Consider the limited services available on weekends (e.g., supplies, formula) when completing necessary paperwork in a timely fashion

  • Consider accessing food pantries and meal programs offered by federal and state programs
Clinical Nutrition: Formulas
  • Clinical Nutrition – Formulas: Infant Comprehensive Formulary, containing:
    • Formulary and substitutions for infant formulas and modules
    • Nutrient comparisons of formulas
    • Thickening Guidelines (If Speech recommends thickening, discuss with Clinical Nutrition to obtain limitations on amount of cereal recommended PO per day.)
    • Formula Assistance Programs and formula company information
