Important information to help you prepare for a medical appointment

Before any medical appointment, it is helpful to prepare the following important information so that you can make the most of the time with your healthcare provider.

You can print this page out and write down your notes, or use this as a checklist to follow and save the information on your phone or somewhere easily available at all times.

You also can use your patient portal (MyCHOP) before or after your appointment, including sending any questions you want to discuss ahead of time to your provider.

Gather the following information before you go

The basics:

  • Date:
  • Department/Specialty:
  • Healthcare provider:
  • Reason for visit: For example, is it your first visit, a follow-up visit, sick visit, etc.

Medications:

For each medication you take, you should know:

  • Medicine name:
  • Dose:
  • How often you take:
  • Other information:
  • Preferred pharmacy address and phone number:

Medical conditions:

You should have a list of your medical conditions and current treatments. This can be through your patient portal or app on your phone if easier.
