The Pennsylvania Homecare Association recognized CHOP's Home Care with the Gold Award in its Best Practices awards program.
June 2013 — Further demonstrating the quality of care delivered by the state’s in-home healthcare and hospice providers, the Pennsylvania Homecare Association (PHA) has recognized Children’s Hospital Home Care with the Gold Award in its Best Practices in Homecare and Hospice Awards program. The award was presented recently at PHA’s Annual Conference and Exposition in State College.
Children’s Hospital Home Care was selected from entries across the state for its initiative “Pediatric Home Ventilator Program Sets the Standard.”
“CHOP Home Care’s Home Ventilator Program supports a safe and smooth transition from the hospital to the home for ventilator dependent children," says Lori Kramer, AVP of CHOP Home Care. "An interdisciplinary team, including inpatient hospital staff, home care clinicians, community agency resources, along with the patient and family, participates. We have been working diligently over the past few years to enhance our program with evidence-based practices that support teamwork, patient safety, quality patient care, and promotes the highest quality of life for our patients and their families.”
PHA established the Best Practices awards in 2010 to showcase the home care and hospice industry as a whole for its exceptional, yet often unrecognized work and contributions to the healthcare continuum. It is estimated that more than one million Pennsylvanians receive home care and hospice services each year and research shows that home care presents significant cost savings to both Medicare and Medicaid compared to institutional-based care.
Children’s Hospital Home Care was selected by a committee of peers, as well as a panel of healthcare experts and state officials.
