Transfer Requests from Outside Hospital, Eligibility for Delayed Outpatient Procedure and Image Transfer Process

Children will be accepted for transfer if the referring clinician prefers that the child/family be evaluated urgently.

Referring Hospital	Ensures upload of images for CHOP Radiology, Orthopedic review Upload Imaging to Ambra — sent to CHOP Radiology Instructions to Upload Imaging Provides family with discharge instructions: Return for Surgery Instructions
CHOP ED Coordinator	Initiates electronic image transfer request Transport Comm Specs assist as needed
EMCP	Ensures OSH uploads images, per instructions, link above Communication with Orthopedics regarding child's eligibility for delayed procedure Enters order for 2nd read of X-ray Completes referral to Orthopedics Use ED Fracture Discharge Smart Set Use referral order © 2023 Epic Systems Corporation
