Pepare for Your Appointment with Immunology

Before your child’s visit  

Before your child’s appointment with Immunology, please send all recent and updated medical records to us by email, MyCHOP or fax. 

Before you leave home, make sure you know where your child’s appointment will be held (address, building and floor), as well as the name of the physician you will see. When you arrive at Children’s Hospital of Philadelphia, you can ask anyone with CHOP identification for help directing you to your child’s appointment.   

Write down a list of questions you want to ask the doctor during the visit. If your child is old enough, help them add questions to the list as well. This will help us address your and your child’s concerns during the visit.  

What to bring to your child’s appointment 

A parent or legal guardian should accompany your child to their Immunology appointment to provide an accurate medical history, consent for the examination and any testing, as well as support for your child. If your child will be brought to the appointment by someone other than a parent or legal guardian, they must bring a letter of authorization (written permission) before your child can be seen.  

The written permission must have the name of the patient, the name of the designated adult and a statement that there is no court order pending. It must be witnessed and signed by two other adults (other than those named on the written permission form).  

If this is your child's first appointment with Immunology, please arrive 30 minutes in advance of the appointment to allow for registration and to review insurance information and related documents. Please allow extra time for traffic and parking as needed. 

Please bring:  

  • The name, phone number and address of your child’s primary care physician; as well as the names and contact information of any other physicians who should receive a follow-up letter from CHOP about your child’s condition.
  • All medications your child will need that day, as well as a complete list of medications and spacers that your child uses. Please bring medication instructions, dosages, etc.
  • Names and phone numbers of emergency contact people.
  • A list of your child's allergies.
  • Insurance information and referral forms if needed. Remember that payment is due at the time of the visit.
  • Toys, books or schoolwork to occupy your child. There will be times when you must wait.  

Your child's evaluation  

Your child’s visit may take longer than you expect, especially if this is your child's first visit.  

The appointment will include:  

  • A complete medical history and physical examination of your child
  • Discussion of your child's diagnosis
  • An outline of the treatment strategy and some educational materials  

By the time you leave the office, you will know your child’s treatment plan. If you have questions, please ask.  

Common questions include: 

  • Will there be another appointment and when?
  • Who do you call if you have questions once you get home?
  • How will information get to your referring physician, other medical specialist or your insurer?  

The members of your child's care team should be able to answer these and any other questions you may have during your first appointment.  

Payment and insurance  

Payment is required at the time of the visit. All co-pays, if applicable, will be collected at the time of the visit. The co-pay established for a specialist is the amount due at the visit.  

Please remember to bring your current insurance card with you. If your child is insured by a managed care plan or HMO, a referral is necessary for your child’s visit to an Immunology physician. The referral should specify the type of visit and whether any testing or laboratory work can be done at the time.

Late policy  

If you arrive late, you risk being seen later or having your appointment rescheduled. Please call us at 215-590-2549 if you are running late.  

Cancellations  

If you need to cancel your Immunology appointment, please give us at least 72 hours’ notice. Another family will be grateful for your appointment time. Please call 215-590-2549 to cancel.
