The Comprehensive Bone Marrow Failure Center (CBMFC) at the Children’s Hospital of Philadelphia (CHOP) is committed to providing comprehensive care for children, adolescents, and young adults with bone marrow failure disorders. Our goal is for our patients to become confident and comfortable in their own health care, so that they and their families are prepared for a smooth and successful transition to adult life when the time comes. To achieve this, we will start discussing transition with patients and families during their teenage years. CHOP strongly recommends transfer of care by the 21st birthday. However, to better meet individual needs, the timing for transfer of care from the CBMFC can vary and includes consideration of the transition policies for other involved CHOP specialty providers, as well as general life milestones, such as completion of a college degree or beginning full-time adult employment.

How to plan to reach this goal:

  • Our physicians, nurse practitioner, psychologist and social worker are here to support you in thinking about your next steps
  • We will continue to be a resource to you while you are in the process of transitioning
  • We will help ensure you feel prepared at the time of transition
  • We will help you identify an adult provider
  • We will coordinate with your other specialists to ensure a seamless transition
  • Your transition will often be directed by your primary specialist team
