New Policy: Helping Families Become Experts in Diabetes Management

The Diabetes Center for Children (DCC) is implementing a new policy for newly diagnosed type 1 patients for their first year of care to ensure that they and their families receive the best, most complete care possible.

What will change

Children diagnosed after July 1, 2014 will receive all of their diabetes care at CHOP’s Main Campus for one year following diagnosis.

This will ensure access to all members of the DCC’s multidisciplinary care team — including dietitian, social worker, certified diabetes educator, nurse practitioner and endocrinologist — and allow for an intensive focus on education and living well with diabetes. After the first year, families have the option of follow-up care at any satellite location.

Questions, concerns or to refer a patient: Call 1-800-TRY-CHOP, press 2, and ask to be connected to one of our endocrinologists to discuss a new referral.
